If you are transferring from another hockey club, you MUST provide a clearance letter.

Please email your clearance letter to admin@milnertonhockeyclub.co.za before or immediately after submitting this registration.

Your registration cannot be approved without a valid clearance letter from your previous club.

If you are registering a player under 18 years of age, a parent or guardian must complete and submit this form on behalf of the player.

I/We understand that personal data will be encrypted and stored securely. * I/The Player agree to abide by the MHC Code of Conduct, which includes respecting all players, coaches, umpires, and officials; maintaining good sportsmanship; and representing the club with integrity both on and off the field. * I/We understand that membership fees are non-refundable once paid. I/We also understand that I/the player participates at my/their own risk, and Milnerton Hockey Club is not liable for any injuries sustained during club activities. I/We give permission for photos and videos taken during club activities to be published on the club's website and social media channels; if I/we wish for any content to be removed, I/we understand I/we may request this from the club. * I/We commit to paying all membership fees, match fees, and any other club-related costs on time according to the selected payment plan. I/We understand that failure to meet payment obligations may result in suspension of playing privileges and that in the event of non-payment, the player may be blacklisted with Western Province Hockey Union (WPHU). *
